Higher CV Event Rate Linked to Low Hb in HD Patients - Renal and Urology News

NATIONAL HARBOR, Md.—Hemodialysis (HD) patients receiving either epoetin or peginesatide who achieve low hemoglobin (Hb) levels have an increased rate of cardiovascular (CV) events, according to new data presented at the National Kidney Foundation 2012 Spring Clinical Meetings.

Pablo E. Pergola, MD, PhD, of Renal Associates in San Antonio, Tex., analyzed data from 1,608 subjects who participated in two phase 3 clinical trials comparing peginesatide with epoetin. Peginesatide is an investigational medication under review by the FDA as a treatment for anemia in dialysis patients. The two trials demonstrated that peginesatide was not inferior to epoetin in maintaining Hb levels in HD patients.

The study by Dr. Pergola's group included 542 patients who received epoetin and 1,066 who received peginesatide. The researchers examined the cardiovascular composite safety endpoint (CSE) rate for both groups of patients. CSE included stroke, myocardial infarction, death, congestive heart failure, unstable angina, and arrhythmia.

For both epoetin and peginesatide, CSE rates were highest among patients with achieved Hb levels of 10 g/dL or lower: 0.90 and 0.64 CSE events/patient-year, respectively. By comparison, the rates were 0.19 and 0.15, respectively, for achieved Hb levels above 12 g/dL.

The CDRG retrospective cohort study compared cumulative incidence of transplant in 1,873 daily home hemodialysis patients using the NxStage System One from 2005 to 2007 and a matched cohort of 9,365 thrice-weekly, in-center hemodialysis patients from the United States Renal Data System (USRDS) database. The study was designed using a unique, rigorous matching process with 17 covariates that could impact transplantation incidence, including demographics, comorbidity and socioeconomic status. Specifically, findings showed:

  • Relative transplant incidence with daily home hemodialysis was 1.14 (P = 0.03) as compared with thrice-weekly in-center hemodialysis, but relative incidence varied by waitlist status at the start of follow-up; and
  • In patients on the transplant waitlist at the start of follow-up, relative transplant incidence was 0.93 (P = 0.27) as compared with thrice-weekly in-center hemodialysis; in patients not listed, relative incidence was 1.91 (P < 0.01);

"Kidney transplantation is widely regarded as the 'gold standard' treatment for End Stage Renal Disease and is associated with the highest quality of life and survival outcomes for patients," said Dr. Allan Collins, professor of medicine, University of Minnesota, and Director, Chronic Disease Research Group. Has a radiologist ever challenged one of your orders for an imaging procedure? - Renal and Urology News
An upcoming issue of Renal & Urology News includes a "Legal Issues in Medicine" column about a urologist and a radiologist who were sued for malpractice. A patient had presented in a hospital emergency department with flank pain and the urologist ordered an excretory urography study for a suspected urinary stone.

The radiologist objected, telling the urologist that unenhanced computed tomography was the standard of care in such cases. The urologist insisted on his way, and the radiologist capitulated. The patient died after going into anaphylactic shock from the intravenous contrast solution.
